Are there any issues in running Composite C1 in a Web Garden environment? I.e. setting the number of concurrent App Pools in IIS to more than 1.

A Web Garden setup would probably work in 'read only' setups, but I would expect problems if you update content using the C1 Console. Data change events would not be raised across processes and you would end up in situations where cached data, like sitemaps, are out of sync .

Is this an issue even if we're using SQL Server as the C1 data store?

Sounds like this will be the same problem in a Web Farm environment, such as Azure. I understand that the next version of Composite will support SQL Azure. With the new version, will Azure and Web Farms/Gardens still need to run in "Read Only" mode?
